If you plan to volunteer to drive students on field trips or on other school related activities you must first register your automobile. Please submit the completed form to the school office.

Did you know you can order lunch for children's field trips? Just fill out the field trip lunch order form and a lunch will be available.
Medication Permit (pdf)
All medications (prescriptions and over-the-counter) must be administered through the Health Office. Please submit a medication permission form, signed by the parent/guardian and physician indicating the name of the student, name of medication, dosage, time of administration, and reason for the medication. The medication must be in its original container labeled with the student’s name and dosage otherwise it will not be given. This form must be renewed annually.

Student Internet Use Contract (pdf)
Before a Los Gatos Union School District student may access the Internet from school, he/she must receive age-appropriate Internet instruction at school and agree to District rules. Please review the Student Internet Use Contract and return the signature page to your child's teacher.
